A. Price Estimates

1. This price estimate is a preliminary calculation and does not constitute an official invoice or a legally binding contract until a formal agreement is executed.
2. All listed prices are final; the Clinic does not negotiate service costs.
3. This document is system-generated and does not require a corporate stamp for validation.
4. This estimate has been prepared specifically in accordance with the treatment plan discussed during the consultation. No modification is permitted without the express written approval of both the attending physician and the Clinic's management.
5. The final cost may vary depending on the complexity of the case, the duration of the procedure, the quantity of materials used, and the location of service. Pricing is determined through an automated calculation based on these variables.
6. The price presented is an approximation generated by an artificial-intelligence system based on initial surgical requirements; the final cost may change once all procedural elements are finalized. Treatment plans and their costs are unique to each patient.
7. This estimate remains valid for no more than thirty (30) days from the date of the client's visit.

B. Payments, Discounts & Fees

1. Full payment for all agreed services is required before any treatment or service begins.
2. Discounts remain valid until the offer period expires or for one calendar month from the date of the client's visit, whichever comes first.
3. For surgical procedures, full payment is due when the surgery date is scheduled, or no later than fifteen (15) days before the procedure, unless an alternative payment schedule is agreed in writing.
4. A non-refundable booking fee is required to secure a surgery date: AED 5,000 for procedures performed at the Clinic's facility, and AED 20,000 for procedures performed in a hospital.
5. The client is responsible for all applicable Value Added Tax (VAT) and any other government-levied taxes related to the services described herein.

C. Scope of Surgical Services

1. A surgical estimate may include:
   • anesthesia consultation fee
   • operating-theatre charges
   • applicable hospital charges
   • one (1) post-operative night in hospital, if medically required
   • up to six (6) follow-up appointments, including wound care.
2. Unless otherwise specified in writing, the estimate excludes:
   • post-operative or surgical garments
   • blood tests
   • radiological examinations (e.g. X-ray, MRI)
   • ECG tests
   • prescriptions and medications
   • PCR or pregnancy tests
   • post-operative lymphatic massage sessions
   • any other item or service not explicitly listed herein.

D. Scheduling, Modifications & Agreement

1. Acceptance of these terms is communicated by telephone to +971 50 144 3888, quoting the estimate number. Upon confirmation, the Clinic will proceed with the services as stipulated.
2. Services requested beyond the scope of this estimate incur additional charges and are contingent upon the approval of the physician and the Clinic.
3. Services are rendered according to the schedule provided by the designated case manager. The Clinic reserves the right to reschedule surgeries for safety or operational reasons.
4. The selection of the hospital facility for any surgical procedure remains at the Clinic's sole discretion. The patient will be formally notified of the designated facility at least five (5) days before the scheduled surgery date.
5. Requested alterations to the services detailed in this estimate are not binding unless accepted and confirmed in writing by the Clinic. Such alterations may incur additional charges, for which the client agrees to be liable upon confirming the changes.

Payment Terms

1. Payment Due Date. Payment is due in full upon receipt of this invoice, unless otherwise stipulated in a separate written agreement.
2. Payment Validity. All payments made are valid for three (3) months from the date of payment, after which they expire.
3. Offers & Discounts. Services booked during a promotional offer, or to which a discount was applied, are strictly non-refundable.
4. Late Payment. In the event of a delay in payment, a late fee of AED 200 per day will be applied for a maximum of seven (7) days. If the invoice remains unpaid after 7 days, scheduled treatments will be automatically canceled, and the full outstanding amount, inclusive of all accumulated fees, becomes immediately due and payable.
5. Payment Methods. Payments may be made via bank transfer, credit card (Visa, MasterCard), or debit card. Bank account details are available upon request.

Cancellation, Rescheduling & Refund Policy

6. Non-Cancellable Services. All services that have been booked and confirmed are considered final, fully payable, and cannot be canceled.
7. Cancellation Policy. If a service is explicitly designated as cancellable, the following notice periods are mandatory:
   • non-surgical services — at least 48 hours before the appointment;
   • surgical services — at least ten (10) days before the surgery date.
   Failure to meet these timelines results in forfeiture of any amounts paid, and the total amount becomes immediately due and payable.
8. Rescheduling Fee. For surgical procedures, a fee of AED 2,500 applies to any request to reschedule a confirmed service.
9. Refund Policy. Refunds are subject to the following conditions:
   • a formal request must be submitted by completing and signing the Clinic's official refund form - no refund is processed without it;
   • no refunds are issued for services partially executed or availed;
   • if a refund is approved, the following deductions apply:
     • an administrative fee of 5% of the total value or AED 700 (whichever is higher),
     • bank processing fees of 5% of the total value or AED 500 (whichever is higher),
     • and any applicable booking fees or non-refundable deposits.
   Approved refunds are processed within 45 days from the date of approval.

Deposits

1. Service Allocation. Deposits are allocated to specific services and cannot be transferred to other services or canceled without the express approval of both the treating doctor and the Clinic's management.
2. Validity. All deposits are valid for three (3) months from the date of payment, after which they expire.
3. Refunds. The Clinic reserves the right to approve or deny refund requests at its sole discretion. No refund is issued for a deposit if any portion of the associated service has been utilized. A client with an unused deposit balance who wishes to obtain a refund may be eligible for a gift voucher equal to the remaining amount, subject to management approval.
4. System-Generated. This is a system-generated deposit record; no signature or stamp is required.

Booking Fees, Rescheduling & Cancellations

6. Booking Fee. A booking fee of AED 200 is applied towards your service and will be deducted from the final invoice for the consultation or treatment.
7. Rescheduling. A confirmed appointment may be rescheduled a maximum of two (2) times. Rescheduling requires a minimum notice of 48 hours before the scheduled appointment.
8. Cancellation. Cancelling an appointment requires a minimum notice of 48 hours. A cancellation fee of AED 100 applies to cover administrative and bank fees.
9. No-Show & Late Cancellation. Failure to provide 48 hours' notice for a cancellation, or a no-show for a confirmed appointment, results in the forfeiture of the full amount paid (booking fee or deposit).
